*Ensure you have your health fund card available at your appointment. If your health fund claims or child’s CDBS allowance has already been utilised else where we may be required to charge the standard rate for treatment. Offers subject to change.
* We have no control over the value of the rebates provided by health funds for the services provided. Services provided and item numbers charged (011, 037, 072, 073, 022, 022, 114, 121) under the Oral health package are fixed. No refund can be offered should your health fund pay more than $250) for services provided as part of the oral health package.
*A minimum $50 deposit is required when prebooking all treatment appointments. This deposit will be forfeited in the case of cancellations within 24 hours, unless an appropriate certificate (i.e medical/carers) is provided.
